Information and Referral Support

Our Information and Referral Support program is coordinated by the Outreach Communication Facilitator (OCF). The role of the OCF and the Information and Referral Support program is to pave the road for effective interaction in the community. The OCF focuses on supporting community members who require facilitation due to a communication disability however other persons who require general information and referral are more than welcome to access the support of the OCF. The OCF will support community members, families and organizations to:
  • provide information on various questions concerning a multitude of topics (housing options, how to apply for programs, etc.)
  • assist in finding specialized programs and resources
  • assist with filling out and submitting applications (ASAC, DF, etc.) 
  • provide facilitation at various appointments including medical, Service Ontario/Canada,ODSP etc. 
  • facilitate large group events that include persons with a communication disability to maintain inclusiveness
  • working with individuals and families on strategies for self-advocacy and self-direction
  • partake in the process of transitioning individuals from LTC into the community
  • assist with other scenarios as they arise, at the discretion of the OCF.
