Information and Referral Support

Our Information, Resource, and Referral (IRR) Support Program assists community members in accessing the services, programs, and resources they need to fully participate in daily life. The IRR Coordinator provides information, guidance, assistance navigating programs, and can connect those with a communication disability to our facilitation services.
The IRR Coordinator supports community members, families, and organizations by:
  • Providing information on a wide range of topics (ex. housing options, how to apply for programs, etc.)
  • Assisting in finding specialized programs and resources
  • Assisting with completing and submitting applications (ex. ASAC, Direct Funding, etc.)
  • Connecting individuals to facilitation services for appointments (ex. medical visits, Service Ontario/Canada, or ODSP meetings)
  • Supporting inclusive participation in large group events that include persons with communication disabilities
  • Working with individuals and families on strategies for self-advocacy and self-direction
  • Assisting with the transition process from long-term care into the community
  • Providing support in other situations as needed, at the discretion of the IRR Coordinator
If you require Information and Referral Supports, please contact us via email or by phone at (416) 222-4448.
